The Logan Hospital Patient Flow Unit is committed to identifying ways that they can improve the services and facilties that they provide to the organisation, consumers, workforce and external providers. As part of this committment the Patient Flow Team are currently undertaking work to identify opportunities to redesign the service delivery and facilities of the Transit Care Hub (previously known as the Transit Lounge).
We are consulting with our workforce to get feedback that will assist us with identifying their current understanding of the services that the Logan Hospital Transit Care Hub provides, their experiences when using the Transit Care Hub services, why they do an don't utilise the service, how they think we can improve the service and facilties to better meet the needs of the workforce, the organisation and the consumer.
This feedback will be used to identify the need for change and how as part of that change we can best meet the needs of the consumer, the organisation and the workforce.
Once the consultation period is closed, the feedback will be collated and formally reported. This report will form part of the project plan and will fedback through apporpriate governance chanels to senior leaders and executives and to the clinical direct care staff.
