The Logan Hospital Transit Care Hub is committeed to providing our consumers, workforce and external providers with a service and facility that meets their needs and provides safe patient focused care. The Logan Hospital Transit Care Hub strives to support effective patient flow through the hospital and improve the patient journey from admission to discharge, transfers and returning for follow up care. As part of the committment to providing these services and facilties, the Patient Flow Unit and Transit Care Hub team are trying to identify how well the Transit Care Hub is meeting the needs of our consumers.
The Logan Hospital Patient Flow Unit and Transit Care Hub team are consulting with the patients, carers and families that utilise the Transit Care Hub services to identify ways we can improve our service delivery and our facilties to better meet the needs of our consumers.
Completing the survey is voluntary, if you choose not to participate it will not affect the level of care you are provided during this admission or future admissions. This survey can be submitted anonymously or if you would like to be contacted to discuss your feedback further you can provide your name and best contact details. These details will be used only for the purpose of this survey and will not be shared with any third party.
The information and feedback provided by consumers as part of this survey will be collated into a report that will be used to inform sernior management and executive leaders of the faciltiy and the hospital health service on what service and faciltiy improvements are required to meet the needs of our consumers. The information will also be utilised to improve the servce and care that our nursing team provide out consumers.
