The Hospital Avoidance and Substitution Health Service Plan is the first comprehensive health service plan for hospital avoidance and substitution services in Metro South Health over the short (5 years) and medium (10 years) term. It is a key directional plan and supports and enables our Strategic Plan.
This Plan was undertaken in two main stages:

This Health Service Plan outlines the current and emerging service needs; identifies the challenges in meeting these needs; and identifies the key service directions and strategies to address the prioritised health service needs for hospital avoidance and substitution services across Metro South Health.
The Plan is provided as a draft for consultation and does not represent Metro South Health policy at this time. No commitment has been made to implement any of the recommended service directions or strategies.
Metro South Health is committed to delivering the best possible services within the catchment and values input from a range of stakeholders including consumers, carers, the community, GPs and other service providers. The purpose of this engagement is to seek feedback on the draft Hospital Avoidance and Substitution Health Service Plan to ensure that our health services meets the current and future needs of our population.
