In the Transition to MAC survey conduced in June 2016, it was identified that there were a number of issues in the way the assessment form had been modified. A workgroup from the MAC User Group reviewed the form, and based on the feedback from the survey, the form was modified and trialled. It was then printed for general use, with evaluation planned for 2 months after commencement.
