Metro South Health and Redland Hospital Heart Failure Service wishes to have your feedback on this service. During 2022 you were either reviewed or assessed by one of the heart failure nurses.  

To ensure we are providing a service that meets the needs of our patients as well as allowing us to improve the service we provide, we are conducting a satisfaction survey.  You are one of approximately 50 clients randomly selected to receive this survey.

Please take a few minutes to complete the following questions that ask about the care you have received from the Heart Failure Service.  Your answers are important as they will help the Heart Failure team in improving the quality of service it provides to patients.  Remember, the survey is completely confidential.  There are no right or wrong answers; it is your own opinion that is important.
